- This application must be completed even if you do not plan on participating in the ceremony.

Ph.D. Forms

Turn form in to Ms. Elizabeth Davis at least two weeks prior to oral exam. It will be sent to the Graduate School electronically. Your major professor will receive the evaluation form from the graduate school to bring to your meeting. - Admission to Candidacy
- Dissertation Proposal
Please print and complete form
Return completed form to Ms. Elizabeth Davis after the successful proposal of your dissertation along with one copy of the proposal. - Application for Graduation
- Announcement of Doctoral Dissertation Defense
Please print and complete. Turn form in to Ms. Elizabeth Davis at least two weeks prior to defense. It will be sent to the Graduate School electronically. - Approval Form for Doctoral Dissertation
